Verizon Fios offered a great service, but not everyone is serviceable for Fios. We needed customers to feel safe providing their address upfront so we could quickly let them know what was available in their area.

On the first screen, I quickly provided an off-ramp for current customers who ended up on this page. Then I used progressive disclosure with the address form, only showing the fields that absolutely need to be shown at each step to avoid overwhelm. I used a simple loading screen animation while we did the address check on the backend to keep people engaged.
